To receive your refund off two (2) jars CLASSICO product AND either the Blu-ray™ or DVD of EAT PRAY LOVE, send all of the following to the address listed below:
1. The original Classico UPCs found on the labels.
2. Original store-identified and dated cash register receipt(s) for Classico purchase and EAT PRAY LOVE purchase with purchase price(s) and item(s) circled.
3. The completed rebate form.
